Serving as a prequel and the fifth main entry in the Mafia series, the story unfolds in early 1900s Sicily. Players follow Enzo Favara, a former child laborer who escapes forced work in a sulfur mine to seek a new life within the Torrisi crime family. As Enzo ascends through the criminal underworld, he becomes entangled in deadly turf wars, complex personal loyalties, and long-standing vendettas alongside figures such as Don Bernardo Torrisi and his daughter Isabella.
Built on Unreal Engine 5, the gameplay emphasizes a focused, linear story structure. Combat combines close-quarters stiletto blade duels, stealth ambushes, and period-authentic firearms like the sawed-off lupara shotgun. Traversal across Sicilian countryside vineyards, opera houses, and underground crypts takes place on foot, on horseback, or in turn-of-the-century motorcars.
